Fees & insurance

Our Standard Rates:

New Patients  

• $350 for the initial "intake" appointment.    

Established Patients  

• $115 for short appointments involving straightforward medication refills.  
• $150 for many appointments involving medication management.  
• $225 and up for appointments requiring more time and/or medical decision making    

These rates are established based on standards in the community.

Financial Policy

We accept insurance from the companies listed below. We ask that you plan ahead to pay at the time of service your co-payment, co-insurance, any deductible not met, or any portion you are responsible for. We are not able to accept "self-pay" only scenarios for services - you must be insured.

• First Choice Health Network

We will make every attempt to assist you in determining what your health insurance plan will or will not cover, but ultimately the responsibility for understanding the coverage falls on the client. Please feel free to contact our office if you have any questions about our financial policies.
